Arch Ophthalmol 2002, 120:431-438. • This study examined 15 eyes of 9 patients who underwent cataract extraction following PRK for myopia (pre-PRK SE -6 to -12 D). The study not only reported the clinical results of the cataract surgeries but also examined 8 different methods of measuring or calculating keratometric diopters for IOL calculation (Table 2) utilizing 3 different lens calculation formulas: (SRK/T, Holladay 1, and Hoffer Q). All 6 eyes whose lens calculations were performed using post-PRK K values had hyperopic surprises (range: +1 to +6 D). Theoretical results indicated that the clinical history method performed best of the 8 methods examined. The study also concludes that the Hoffer Q formula, combined with the clinical history method affords the most accurate results.

The results showed that the Gaussian optics formula performed better than standard topography (EffRP) and keratometry (K) derived measurements. Linear regression analysis demonstrated that the accuracy of EffRP based calculations of postoperative keratometric diopters can be greatly improved by reducing the value by 0.15 D for each diopter of LASIK-induced refractive correction. Similarly, the accuracy of K based calculations can be improved by reducing the value by 0.24 D for each diopter of LASIK-induced refractive correction.
