J Perinatol 2001, 21:415-420. • This paper describes a historic cohort study of neonates who received platelet transfusions over a 41-month period at a single hospital in Mexico City, Mexico. Despite the fact that complete records were available on fewer than 50% of the transfused babies, the findings of the study are very similar to those conducted in the United Kingdom (Murray et al. [14]) and U.S. (Del Vecchio et l. [16]) at around the same time. In particular, babies receiving platelet transfusions had a higher mortality than those who were not transfused (24.5% vs 3.7%), and the only reported hemorrhage was IVH, which is likely to have antedated the thrombocytopenia in the vast majority of babies.

Am J Obstet Gynecol 2001, 185:683-688. • This is the first report of a large population of patients (n=56) at risk for severe fetal alloimmune thrombocytopenia (as a result of a previously affected sibling) treated at a single center comparing the effects of invasive management (fetal blood sampling ± fetal platelet transfusion) with a noninvasive strategy (no fetal blood sampling). The eight pregnancies in which a previous sibling had suffered an ICH were managed invasively (fetal blood sampling ± fetal platelet transfusion and maternal IVIG) with a good outcome in all cases. The remaining 48 pregnancies (previous affected sibling had severe thrombocytopenia without ICH) were divided into 2 groups to assess the effects of invasive versus noninvasive treatment (maternal IVIG alone). No ICH was seen in either group, and the authors conclude that invasive management should be restricted to pregnancies in which previous sibling(s) suffered ICH.

Only 27 verified cases of NAITP were identified in 18 families (1 in 16,500 live births), which is far below the expected incidence (1 in 1,000-1 in 2,000). This suggests that NAITP is underdiagnosed and that increased clinical awareness and possibly antenatal screening programs may be needed to avoid preventable deaths and disability due to severe thrombocytopenia.

The RNA was extracted from intestinal segments removed at surgery from 21 neonates with necrotizing enterocolitis, 18 neonates with other acute inflammatory conditions, and 9 neonates without intestinal inflammation. Using semiquantitative RT-PCR, the authors found increased levels of IL-11 mRNA in the neonates with necrotizing enterocolitis, increased IL-11 mRNA, and decreased likelihood of pan-necrosis, suggesting that local upregulation of IL-11 might represent an adaptive response to limit the extent of intestinal damage.

It establishes that plasma IL-11 is below detectable limits in normal healthy neonates and that raised plasma levels of IL-11 are strongly associated with sepsis and necrotizing enterocolitis. This suggests that IL-11 is involved in the endogenous cytokine response to sepsis and necrotizing enterocolitis in preterm neonates.
