    • Utility of standard nutritional parameters in detecting body cell mass depletion in patients with end-stage liver disease

Liver Transpl 2000; 6:575-581. In order to determine which of the traditional nutritional parameters correlate with BCM, several standard parameters were assessed in 69 sequential patients awaiting OLT. Most of the commonly measured nutritional parameters correlated poorly with BCM. Mid-arm muscle circumference and handgrip strength were the best predictors of depleted BCM in a multivariate analysis. These two parameters combined were able to identify BCM depletion with a sensitivity of 94% and a negative predictive value of 97% when handgrip strength was less than 30 kg and mid-arm muscle circumference less than 23 cm.

Malnutrition in patients with hepatic cirrhosis is associated with abnormal fuel metabolism marked by reduced glucose and increased lipid oxidation. In 15 cirrhotic patients awaiting OLT, the respiratory quotient, assessed by indirect calorimetry, was correlated with classic anthropometric nutritional parameters. The respiratory quotient did not correlate with any of the routine nutritional parameters, except for serum albumin and creatinine-height index, although the association was poor. Therefore, anthropometric measurements may suggest normal nutritional status while indirect calorimetry suggests abnormal fuel metabolism. Metabolic derangement In cirrhotic patients may precede rather than follow PEM.

In this study a subgroup of 72 patients awaiting OLT had BIA and DEXA measurements of fat and fat-free masses in pre- and posttransplant periods. BIA data correlated well with DEXA values and was able to demonstrate fat and fat-free masses differences before and after liver transplantation. The use of BIA in these patients can improve the monitoring of nutritional status by allowing frequent and timely evaluation of body composition changes, which will help early intervention and adaptation of nutritional care of transplant patients.

Poor preoperative nutritional status has been reported to be associated with adverse outcomes after OLT. In this prospective study the relationship of a wide array of preoperative nutritional parameters with different postoperative outcomes such as survival, intensive care unit and hospital length of stay, transfusion requirements, infection, graft rejection and global resource utilization (as an index of cost) were investigated in 53 patients undergoing OLT. Despite minor statistical significant associations, neither survival nor global resource utilization were associated with any preoperative nutritional parameter. Therefore, adverse outcomes after OLT may be primarily determined by factors others than the preoperative nutritional status. However, the results may also suggest that preoperative nutritional support, which was offered to this malnourished population, was able to overcome the adverse effects of malnutrition on OLT outcomes.

Early liver post-transplant outcomes such as 1-year survival, ventilator time, and intensive care unit and hospital lengths of stay were measured in 80 adult patients undergoing OLT. These parameters were correlated to nutritional indicators (handgrip strength, triceps skinfold thickness and uncorrected mid-arm muscle area) to determine the predictive value of such nutritional parameters on postoperative outcomes after OLT. Relationships between nutritional indicators, when controlled to Child-Pugh class, and early morbidity could not be determined due to insufficient data. No relationship was established between one-year survival Child-Pugh class or nutritional indicators.

In this interesting study 82 listed candidates for OLT with mid-arm muscle circumference less than the 25th percentile were randomly assigned to receive enteral supplementation (750 kcal, 20 g protein and 33.5 g fat) in addition to conventional diet or conventional diet alone, until they were transplanted. Supplementation improved mid-arm muscle circumference and handgrip strength, but did not affect the outcome. Despite more deaths in the control group, the difference in overall survival between the groups was only marginally significant (P=0.075).
