This study demonstrated that cocaine abstinence among treatment-resistant cocaine-abusing heroin addicts can be increased by increasing the value of vouchers used in a voucher-based contingency management therapy. Reinforcement magnitude was a critical determinant of the effectiveness of this abstinence reinforcement intervention.

This is a review of the advantages and disadvantages of the heroin trials and the history of heroin prescription in the UK. An interesting discussion is provided of the conclusions that can be drawn from the results of the Swiss Heroin Trial and requirements for new trials and methodologic difficulties that they will face. This brave article advocates that, if high-quality empiric evidence is found, then a possible medicalization, rather than legalization, of heroin may not only be justified but advisable.

In a 40-week randomized double-blind trial at Johns Hopkins, urine analysis and retention in treatment was assessed in 97 patients allocated to moderate doses (40-50 mg/day) and 95 patients to high doses (80-100 mg/day) of methadone. The same group had previously shown that moderate doses were more effective than low doses (20 mg/day).

This study evaluated the success of a new treatment requirement that patients who had been maintained on methadone for 12 months gain employment (20 h/week, paid or volunteer) using a behavioral contingency paradigm. Of the patients 75% secured employment and maintained the position for at least 1 month. Most patients (78%) continued working throughout the 6-month follow up. These results demonstrate that behavioral contingencies can motivate many methadone maintenance patients to obtain employment.

This report describes an open study that compared the efficacy of buprenorphine versus methadone in 60 opiate addicts over a 24-week period. Both drugs were effective for some patients. The retention rate was significantly better in the methadone maintained group, however, whereas those in the buprenorphine group had significantly lower rates of illicit opiate consumption.

This study, a substudy of the 'Swiss Heroin Trial', compared two groups of heroin addicts recruited from the community who were socially marginalized and in poor health and had failed in at least two previous drug treatments. Twenty-seven individuals received intravenous heroin treatment as outpatients for 6 months, whereas the control group of 24 individuals received conventional drug treatment. At the end of 6 months, only one patient in the heroin group, and 10 patients in the control group continued to use street heroin. Those in the heroin group had larger improvements in psychiatric health measures, social functioning, and illegal activities related to drug use.

Results indicated a significant, dose-related reduction in cocaine's stimulant and euphoric effects, in addition to a reduction in the desire to use cocaine. In addition to adding to the growing body of literature indicating an important role for the brain DA system in the rewarding effects of abused drugs, these results hold promise for pharmacological treatments approaches for cocaine addiction.
