In search of an explanation of the more aggressive course of PR3-ANCA-associated glomerulonephritis in comparison with the course of MPO-ANCA-associated glomerulonephritis, the authors studied the in-vitro neutrophil-activating capacity of PR3-ANCA and MPO-ANCA. IgG fractions were obtained from patients at the time of diagnosis. IgG fractions from PR3-ANCA positive patients were more potent inducers of the respiratory burst and degranulation than IgG fractions from MPO-ANCA positive patients. These in vitro findings may underlie the differences in clinical presentation between PR3-ANCA and MPO-ANCA positive patients.

Some epitopes were lost after trypsin-EDTA digestion and after glutaraldehyde fixation. Using RT-PCR, however, proteinase 3 was not detected in cultured endothelial cells, suggesting that PR3-ANCA may act as an antiendothelial cell antibody. The authors postulate that PR3-ANCA reacts with the endothelial membrane by cross-reacting with membrane-bound proteins.
