Pediatr Res 1999; 46:305-310. To our knowledge, there is no clear evidence from human neonates to demonstrate that early aggressive nutritional support can decrease morbidity. However, guinea pig pups exposed to hyperoxia had increased survival and liver glutathione concentrations when given TPN compared with dextrose alone.

Methionine concentrations in commercially available TPN solutions are high to compensate for low cysteine concentrations. If methionine proves to be hepatotoxic, as shown in this study, there is thus an urgent need to determine the parenteral methionine requirement, and to determine whether the addition of cysteine could ameliorate the complications induced by high methionine concentrations.

This study demonstrated very significant metabolic differences in arginine metabolism between intravenous and intragastric feeding. It showed that arginine is dependent upon small intestinal metabolism, which when by-passed, as in TPN, must increase dietary arginine requirements.
