Angiotensin-converting enzyme gene polymorphism is associated with coronary atherosclerosis and myocardial infarction in a sample of Italian patients. Eur J Clin Invest 1998; 28:485-490. Although this recent study of the prevalence of the ACE DD genotype did not observe an association between myocardial infarction and the ACE DD genotype in 394 Italian patients undergoing coronary angiography, the results were homogeneous with the slight overall association observed in European countries. Data from this study were included in the Mantel-Haenszel calculation of the overall myocardial infarction odds ratio of 1.20 for DD versus DI + II (Table 1).

Atherosclerosis 1999; 142:211-216. This recent study of the association of the ACE DD genotype with myocardial infarction and angiographically confirmed stenosis recruited individuals from Toulouse, France, which was the source of one of the ECTIM study populations in which the original observation of increased risk for myocardial infarction in ACE DD patients was observed. The results, while negative, were homogeneous with those observed in other European countries and data from this study were included in the Mantel-Haenszel calculation of the overall European odds ratio for myocardial infarction (Table 1).

Atherosclerosis 1998; 139:153-159. This large case-control study of coronary angiography results from 2267 male German Caucasians is an expansion of an earlier study 920 individuals reported in 1995. The expanded study was undertaken because power analysis indicated that larger samples sizes were needed to detect ACE-DD associated differences in risk for CHD. The study identified the D allele as a risk factor for coronary artery disease in young individuals and for myocardial infarction in older individuals. Data from both age groups are included in the Mantel-Haenszel calculation of European odds ratio for myocardial infarction.

Angiotensin-converting enzyme genotypes and risk for myocardial infarction in women
-
41 Anderson JL, Carkquist JF, King GJ, Morrison L, Thomson MJ, Ludwig EH, et al. Angiotensin-converting enzyme genotypes and risk for myocardial infarction in women J Am Coll Cardiol 1998; 31:790-796. This case - control study of coronary angiography results from 490 American women treated at the LDS Hospital in Utah is an extension of the earlier study done by Ludwig et al. [40]. Power analysis indicated that, due to the lower incidence of CHD in women, a larger sample size was necessary to detect ACE-DD associated differences in risk for CHD in women. Because the data in this paper included the women reported on in the prior study, it was used instead of the data from women in [40], and only the data on male patients from that study were utilized in the calculation of combined odds ratio in Tables 1 and 3.

Angiotensin-converting enzyme gene polymorphism is associated with coronary disease in non-insulin-dependent diabetic patients evaluated for 9 years. Metabolism 1998; 47:1258-1262. This Finnish study details the incidence of CHD in 83 newly diagnosed NIDDM patients at the beginning and end of a 9 year observation period. ACE DD genotype was shown to be positively associated with both overall CHD status at the end of the 9 year period and the development of CHD in individuals who were CHD free at the start of the study.

Meta-analysis of association of insertion/deletion polymorphism of angiotensin-I converting enzyme with diabetic neuropathy and retinopathy. Diabetologica 1998; 41:47-55. This meta-analysis of 18 studies of 4773 diabetic patients used the Mantel-Haenszel method to calculate an overall odds ratio for diabetic nephropathy of 1.32, confidence interval 1.15-1.51, for the DD+DI versus the II ACE genotypes, which was significant for both NIDDM patients (P<0.005) and for insulin-dependent diabetics (P<0.05). No statistically significant association of the I/D polymorphism with diabetic retinopathy was observed.
