A plant sterol mixture, containing sitosterol, sitostanol, campesterol and campestanol (62%, 21%, 16% and 1%, respectively) were fed (21.2 mg/kg) in mostly insoluble form to normocholesterolemic individuals for 10 days. Total cholesterol was significantly decreased to 4.7 ± 0.3 mmol/l from 5.0 ± 0.3 mmol/l, and LDL cholesterol to 3.0 ± 0.3 mmol/l from 3.2 ± 0.3 mmol/l by tall oil sterols versus placebo. Plant sterol/cholesterol ratios were not changed and cholesterol synthesis appeared to increase only in individuals with a low baseline cholesterol level.

Twenty-four children with familial hypercholesterolemia North Karelia (FH-NK) deletion, four parents with FH-NK, 16 nonaffected family members and 12 FH-NK adults with simvastatin treatment consumed stanol ester margarine for 3 months. LDL cholesterol was reduced significantly by 17%, 12% (NS), 13% and 20% in the four groups, respectively. HDL cholesterol and serum triglycerides were unchanged. Serum plant sterol/cholesterol ratios were significantly decreased.

Seventy-two healthy, normolipidemic 6-year-old children on a low-fat low cholesterol diet consumed sitostanol ester margarine for 3 months (1.46 g stanols/day) Total and LDL cholesterol levels were significantly diminished without any change in HDL cholesterol, triglycerides or safety markers.

In 22 postmenopausal women with coronary artery disease and on a low cholesterol and fat diet, sitostanol ester margarine (3 g stanol/day) reduced total and LDL cholesterol levels significantly by 13% and 20% from home values. None of the women had LDL cholesterol levels below 100 mg/dl (2.6 mmol/l) during home diet compared with 32% of individuals during sitostanol ester margarine use. In another group of 10 postmenopausal coronary women taking simvastatin, the combination of sitostanol ester margarine to simvastatin reduced total and LDL cholesterol by 11% and 16%, respectively, from simvastatin only period. This observation suggests that the combination of stanol ester and statin therapy potentiates the lipid-lowering effect.
