1. Madersbacher S, Djavan B, Marberger M. Minimally invasive treatment for benign prostatic hyperplasia. Curr Opin Urol 1998; 8:17-26. This is an extensive and thorough review of minimal invasive treatment of benign prostatic hyperplasia. Important information of the most common technologies is presented in three categories: durability of therapeutic response, randomized trials and the impact on bladder outflow obstruction. An excellent paper for those interested in an overview of the current status in endo-technology in urology.
2. Roehrborn CG. Accurate determination of prostate size via digital rectal examination and transrectal ultrasound. Urology. 1998; 51 (Suppl 4A):19-22. This study to compare volume measurements with rectal examination and imaging indicated the underestimation of size by rectal examination, especially in larger prostates. The solution is sought in the improvement of the rectal examination to overcome the additional costs of ultrasound imaging during the investigation.

4. Standaert B, Alwan A, Nelen V, Denis L. Prostate volume and cancer in screening programs. Prostate 1997; 33:188-94. An interesting publication elaborating on the relation between prostate volume and prostate cancer detection. Although the complicated numerical manipulation might obscure the message, the conclusion that the use of uniform screening criteria is difficult to accept because of differences in prostate volume is valid.

The authors report the use of PSAD as a pretreatment screening parameter to predict response to medical BPH therapy. PSAD seems to be an appropriate additional screening parameter in prescribing medical therapy for BPH.

Prostatic volume and ratio of free-to-total prostate specific antigen in patients with prostatic cancer or benign prostatic hyperplasia. J Urol 1997; 158:2188-2192. It was shown that the ratio of free-to-total PSA is influenced by prostate volume, suggesting that it is appropriate to add the principle of density measurements to free PSA investigations as well.

The effect of prostate volume, age, total prostate specific antigen level and acute inflammation on the percentage of free serum prostate specific antigen levels in men without clinically detectable prostate cancer. J Urol 1998; 159:1234-1237. A strong correlation between free PSA level and age and prostate volume is reported in this study, which suggests that functionally percentage free PSA reflects the extent to which an elevated total PSA level is caused by BPH.

This study compared benign prostatic enlargement parameters with symptom parameters and obstruction parameters. It was concluded that these parameters are at most only weakly related.

Data from the subgroup indicates that age is one of the most important parameters that influence the relationship between objective and subjective parameters.

This is a study to identify the number of false-negative results in patients during their first biopsy of the prostate. A cost-effective improvement of the biopsy strategy is proposed by taking two sessions of sextant biopsies in a single office visit. It would be interesting to see how this compares with the diagnostic accuracy when increasing the number of biopsies instead of sextant biopsies, as biopsies of other regions can be added to the process.
37. Svetec D, McCabe K, Peretsman S, Klein E, Levin H, Optenberg S, Thompson I. Prostate rebiopsy is a poor surrogate of treatment efficacy in localized prostate cancer. J Urol 1998; 159:1606-1608. This repeat biopsy study in radical prostatectomy specimen after surgery indicated the large number of false-negative results with sextant biopsies. Although the biopsies of resected prostates do not represent the real clinical situation, the message is that sextant biopsies are inaccurate representations of the presence of prostate cancer, also after localized therapy.
