An outbreak of toxic encephalopathy caused by eating mussels contaminated with domoic acid

Perl TM, Bedard L, Kosatsky T, Hockin JC, Todd EC, Remis RS. An outbreak of toxic encephalopathy caused by eating mussels contaminated with domoic acid. N Engl J Med 1990; 322:1775-1780. This and [8•] are important papers documenting the neurologic effects of poisoning by domoic acid, a structural analogue of kainic acid and glutamate. These observations in humans provide strong evidence for the concept that excitotoxicity accounts for some of the pathophysiological features seen in patients with temporal lobe epilepsy.

Intravenous valproic acid administration in status epilepticus
-
Czapinski P, Terczynski A. Intravenous valproic acid administration in status epilepticus [in Polish]. Neurol Neurochirurg Polska 1998; 32:11-22. Response of status epilepticus to drug treatment is usually much more favorable in open-label, uncontrolled studies (as it is in this study) than in rigorously designed clinical trials (e.g. [29••]). More research with intravenous valproate is required to validate these authors' results.

-
Stecker MM, Kramer TH, Raps EC, O'Meeghan R, Dulaney E, Skaar DJ. Treatment of refractory status epilepticus with propofol: clinical and pharmacokinetic findings. Epilepsia 1998; 39:18-26. The small sample size of this retrospective study limit the authors' ability to detect differences in important outcome measures such as hypotension and time from discontinuation of treatment to extubation. Some hold the opinion that propfol is favored over pentobarbital with regard to these measures. Confirmation from controlled trials is lacking, however.

A single-blind, crossover comparison of the pharmacokinetics and cognitive effects of a new diazepam rectal gel with intravenous diazepam
-
Cloyd JC, Lalonde RL, Beniak TE, Novack GD. A single-blind, crossover comparison of the pharmacokinetics and cognitive effects of a new diazepam rectal gel with intravenous diazepam. Epilepsia 1998; 39:520-526. The authors demonstrate that the rate of diazepam absorption from the gel formulation is sufficiently rapid to suggest utility in the management of status epilepticus. What is still unclear is whether the rate of drug absorption differs between rectal administration of the gel versus rectal administration of the intravenous solution.
