Neuron 1996, 16:77-87. In this rare X-linked dominant disorder, women present with partial epilepsy and sometimes with mental retardation of variable severity. The condition is lethal in male fetuses. Periventicular heterotopia is seen on magnetic resonance imaging. Linkage was found to Xq28, which is the first step to cloning the gene.

Nature Genet 1995. 11:201-203. The molecular genetic basis of autosomal dominant nocturnal frontal lobe epilepsy in one large family was identified. A single base mutation converted a serine to a phenyalanine residue in the second transmembrane domain of the α4 subunit of the neuronal nicotinic acetylcholine receptor. This serine residue is highly conserved throughout evolution and is essential for normal functioning of the ion channel.

Family history is the key to diagnosis, but the subtle nature of the condition in relatives can obscure the diagnosis. The clinical features are compared with those of the well known syndrome of mesial temporal lobe epilepsy caused by hippocampal sclerosis.
