Cytokine 1998, 8:596-602. Extracellular matrix proteins induced the expression of G-CSF in macrophages as did the direct contact to a G-CSF-dependent leukemia cell line. Both effects could be blocked by pretreatment with monoclonal antibodies against integrins and adhesion molecules. Thus, adhesion itself may activate neutrophils via growth factor expression.

However, observation time was short (3 hours), thus not allowing conclusions about possible long-term effects of G-CSF in endotoxin-induced lung injury.

Moreover, suppression of the production of IFN gamma seemed to be one mechanism by which G-CSF exerted its protective effect in LPS-induced shock.

Although mice made neutropenic with 5-fluorouracil benefited from G-CSF treatment of Aspergillus fumigatus infection, mice immunosuppressed with corticosteroids had the opposite outcome. Moreover, in the latter group, G-CSF strongly antagonized the activity of a new antifungal triazole which, when given alone, reduced fungal growth and prolonged survival.

When G-CSF was given 2 hours before infusion of LPS to human volunteers, there was a shift to a relative anti-inflammatory cytokine response compared to the pro-inflammatory cytokine response with G-CSF pretreatment 24 hour earlier. Despite differences in cytokine response, both regimens reduced pulmonary sequestration of PMNs.

In this retrospective review of patients who underwent autologous bone marrow transplantation, 6 out of 46 (13%) of those who received GM-CSF, versus 38 out of 95 (40%) of those who did not receive this cytokine had infection. They speculate that GM-CSF may enhance the function of tissue macrophages. A prospective, concurrently controlled study is required to confirm this finding.
