Precipitation of overt encephalopathy in the portacaval shunted rat: Towards the development of an adequate model of chronic portal systemic encephalopathy
-
Mullen KD, Roessle M, Jones DB, Grun M, Jones EA. Precipitation of overt encephalopathy in the portacaval shunted rat: towards the development of an adequate model of chronic portal systemic encephalopathy. Eur J Gastroenterol Hepatol 1997; 9:293-298. This publication should receive the widest possible attention. Although not including amino acid data, it creates the relevant background against which hyperammonaemia-associated changes in amino acid metabolism should be viewed. The induction of overt encephalopathy in portacaval shunted rats necessitated additional procedures such as an impairment of liver function and blood gavaging. The difference between PSE and HE is not specifically addressed but is implicit in the paper. Experimental models of acute-on-chronic PSE/HE are described.

Proton magnetic resonance spectroscopy of the brain in symptomatic and asymptomatic patients with liver cirrhosis

Gastroenterology 1997; 112:1610-1616. An informative description of glutamine accumulation and myoinositol depletion in the brains of patients with liver cirrhosis, where these metabolic changes occur already in the absence of subclinical encephalopathy. The question of whether in severe clinical HE the marked further increase in brain glutamine may contribute to the pathogenesis, can not be answered on the basis of this study. Information about conventional biochemical data of the patients would have been desirable.

Effects of simulated upper gastrointestinal hemorrhage on ammonia and related amino acids in blood and brain of chronic portacaval-shunted rats
-
Olde Damink SWM, Dejong CHC, Deutz NEP, Soeters PB. Effects of simulated upper gastrointestinal hemorrhage on ammonia and related amino acids in blood and brain of chronic portacaval-shunted rats. Metab Brain Dis 1997; 12:121-135. A clinically relevant, experimental study focused on cerebral cortex glutamate content after intragastric blood administration in PCA rats. The blood gavages induced a deterioration of the neurological state without altering brain glutamate concentration. In addition to ammonia and glutamine, substances such as GABA, phenylalanine and tyrosine were markedly increased in brain tissue, whereas taurine was decreased. The authors suggest that future research should concentrate on releasable neurotransmitter pools.

Butterworth J, Gregory CR, Aronson LR. Selective alterations of cerebrospinal fluid amino acids in dogs with congenital portosystemic shunts. Metab Brain Dis 1997; 12:299-305. The pathogenesis of HE in dogs with congenital PSS is generally thought to be largely analogous to that of HE in human cirrhosis, because in these animals PSS is associated with distinct liver insufficiency. Increased amounts of glutamine, glutamate, and aromatic amino acids were found in the cerebrospinal fluid.

1 improved amino acid-nitrogen economy. A decrease in plasma amino acids and in urea-nitrogen synthesis rate as well as a positive total body nitrogen exchange were observed, with at least one of the underlying mechanisms probably being operative at the peripheral level (reduced amino acid flux to the liver).

Treatment of cirrhotic rats with L-omithine-L-aspartate enhances urea synthesis and lowers serum ammonia levels

Gebhardt R, Beckers G, Gaunitz F, Haupt W, Jonitza D, Klein S, Scheja L. Treatment of cirrhotic rats with L-omithine-L-aspartate enhances urea synthesis and lowers serum ammonia levels. J Pharmacol Exp Ther 1997; 283:1-6. This study shows that ornithine aspartate, although enhancing urea formation, may beneficially influence nitrogen metabolism by promoting protein synthesis. Both of these effects are likely to contribute to the ornithine aspartate-induced reduction in the plasma ammonia level.
