A Southwest Oncology Group Study. Blood 1997, 89:3323-3329. Very large study (211 patients) of elderly AML patients enrolled on single clinical trial. Very high frequency of MDR1 expression (71%) in striking contrast to a frequency of 35% MDR1 expression in younger AML patients recently reported by the same group. Multivariate analysis showed CR rate was highly significantly and independently associated with MDR1 expression (P = 0.0041), secondary disease (P = 0.0035) and unfavorable cytogenetics (P = 0.0031). Resistant disease was highly significantly associated with MDR1 expression. There was no effect of MDR1 on overall or disease-free survival. These findings suggest that the differences in clinical response found in elderly compared to younger patients with AML may in part be related to differences in the biologic behavior of the disease.

Clin Cancer Res 1997, 3:1419-1425. A large clinical study (80 patients) examining the impact of MRP expression on clinical outcome. Patients were not treated with identical protocols but almost all (64 patients) received daunomycin plus standard-dose cytarabine with or without the addition of etoposide. MRP was detected at low (19%), intermediate (55%), or high levels (26%) in the 80 patients. No association between MRP expression and CR rate, overall survival, or disease-free survival. In contrast, high MDR1 expression was associated with decreased CR rate, OS and DFS.
