This is a prospective study of cognitive and psychosocial measures in an unselected sample of acute psychotic patients undergoing a cognitive behavioural intervention known as compliance therapy. Surprisingly, there were few cognitive predictors of gains in compliance and insight despite general improvements in neuropsychological and symptom measures.

In contrast to authors considering lack of insight from either cognitive or motivational viewpoints, this author states that both approaches might be partially correct. Quadratic rather than linear relationships between insight and cognitive deficits are hypothesized, and found. The author suggests that this curvilinear relationship might be mediated by specific brain structures associated with the frontal lobes.

The author points out that mental functions are not accomplished by discrete 'centres' in the brain, but rather by a system of areas. On the other hand, because of anticipation (top-down) processes, apparently similar tasks may differ in their meaning to different individuals and so may be involved in different brain processes. Therefore, the authors of cognitive neuropsychiatric studies should be concerned with the interpretation of their data; making simple analogies between the processes taking place in different conditions may be misleading.

The authors provide a detailed overview on functioning of neural circuits and consider different psychopathological conditions from the viewpoint of disordered cortico-subcortical circuitry. This approach successfully demonstrates the disordered relationships in orbitofrontal circuits (cortex caudate-globus pallidum thalamus-cortex) underlying obsessive-compulsive disorder. In the case of schizophrenia, the question arises as to whether there is a synchronous decrease of cortico-subcortical activation, or imbalance of reciprocal inter-relationships in cortico-subcortical circuits.

The authors argue that recent neuroimaging studies point to disordered functional connectivity between frontal and temporal areas. This disconnectivity is not necessarily understood as a disruption of anatomical interconnections, it is viewed as a specific (abnormal) pattern of physiological activity in different brain areas. The implications of the disconnectivity concept for the understanding of psychotic phenomena are proposed.

The authors offer a unifying approach to the different manifestations of schizophrenia: cognitive deficits, abnormalities in posture, gait, emotional response and expression. All these signs of schizophrenia could be considered as a manifestation of 'dysmetria', that is, considered more generally rather than a solely motor process. The concept of 'cognitive dysmetria' is proposed as the dysfunction of neural circuitry that coordinates the interactions between perception, retention, retrieval and response. The results of a PET study of 14 schizophrenic patients confirm the prediction of dysfunction in prefrontal thalamic cerebellar circuitry revealed by recall of narrative material.

In this study, the authors provide a thorough analysis of the concepts relating to thought disorder in schizophrenia. The results of their experiment demonstrate intact automatic intralexical semantic priming but possibly evidence for disorder of higher level semantic access functions. The authors therefore argue that prior reports of enhanced priming in schizophrenia might have been confounded by methodological problems. The role of neuroleptic medication in abnormalities in priming, previously reported in the literature, is thoroughly tested.

The author reviews investigations of schizophrenic psychopathology using the semantic associative network approach. The pathology of such a network is revealed in a variety of experiments with indirect semantic priming. The results of a cognitive event-related potentials study demonstrated the absence of the N400 wave in thought-disordered schizophrenic individuals when facing nonrelated words (reflecting an abnormality of associative networks). Data on tasks with concrete versus abstract priming, and experiments using dopamine agonists which (because of a hypothesized increase of the focus of activation) appear to decrease the spread of activation within semantic networks in normal individual, add to the theory's implications for schizophrenia. Spitzer argues that the results of his experiments prove the excessive spread of activation between the lexical connections in semantic memory of schizophrenic patients with thought disorder.

In this study, different groups of schizophrenic patients were investigated: first episode unmedicated, multiepisode unmedicated, medicated. Patients with mood dosorders served as a control group. The impairment revealed in unmedicated schizophrenic patients performing CPT was interpreted as the deficit in the processing of contextual information, which could account for other cognitive impairments observed in schizophrenia (i.e. deficits in working memory and in inhibition).

The authors studied the ability to mentalize in 46 symptomatic schizophrenic patients and 44 normal control individuals. Patients with paranoid delusions demonstrated the poorest performance on theory of mind tasks. The same deficits in the patients with negative features or incoherence were caused by memory impairments. Patients with delusions of control and patients in remission did not differ from normal control individuals. The authors concluded that the mentalizing deficit was distant from other cognitive impairments.

The authors hypothesized that any measure reflecting the disorganization of speech planning process might be associated with the proneness to verbal hallucinations. The task required first listening of the preamble that comprised sentences either in active or in passive voice. Then the subjects were presented with a picture and asked to describe its content according to the newly indicated voice. Schizophrenic patients with verbal hallucinations were found to be less accurate and slower than were normal control individuals in this grammatical task. Although the authors could not rule out the possibility of a confounding effect of generalized attention deficit, they concluded that their results might reflect the speech planning disturbance involved in production of verbal hallucinations.

Apart from this finding, performance on the WCST was particularly poor in a subgroup of patients who demonstrated delayed habituation or dishabituation, or both. The authors hypothesized that skin-conductance orienting response abnormalities and poor performance on the WCST were mediated by the same pathology of prefrontal cortex.
