The emphasis is on the historical, but the article also traces the concepts behind the application of intravesical capsaicin for the treatment of detrusor hyperreflexia.

This is a report of a large, dual centre European study on the use of intravesical capsaicin for the treatment of detrusor hyperreflexia. A total of 79 patients, who were unresponsive to oral anticholinergic therapy, were treated. Overall, 80% of patients derived some clinical benefit, although only 44% were able to achieve complete continence. A number of patients in this study have been followed up for 5 years and received multiple instillations of intravesical capsaicin with good clinical effect.

The authors found substantial differences in urodynamic effect between resiniferatoxin and capsaicin and suggest that further studies are required.
