Patients with cluster headache showed abnormalities of brain and skeletal muscle energy metabolism comparable with those seen in various types of migraine, thus leading the authors to suggest a similarity in biochemical pathogenetic mechanisms between CH and migraine.

According to recent studies with evoked potentials, fundamental, probably protective, feature of cortical information processing, namely response habituation during stimulus repetition, is abnormal in migraine between attacks in different sensory modalities and experimental paradigms. The author suggests that this may play a pivotal role in migraine pathogenesis, in conjunction with the reported decrease in brain mitochondrial energy reserve, by favouring a rupture of metabolic homeostasis and biochemical shifts capable of activating the trigeminovascular system, and thus capable of producing a migraine attack.

This study demonstrates an increase in plasma CGRP during a nitroglycerine induced cluster headache but not during the latent period or at the onset of the attack. This suggests that a hyperactivity of trigeminal non receptive fibres could make the trigeminovascular system of cluster headache patients sensitive to the triggering action of nitroglycerine.
