    • Holland NR, Stocks A, Hauer P, Cornblath DR, Griffin JW, McArthur JC. Intraepidermal nerve fiber density in patients with painful sensory neuropathy. Neurology 1997; 48:708-711. This study used punch biopsies of the skin to quantify the density of the epidermal innervation that is derived from unmyelinated afferents. It is shown that there is a reduction in the innervation density in painful neuropathies compared with normal control individuals.

    • Dibhajj SD, Tyrrell L, Black JA, Waxman SG. NaN, a novel voltage-gated Na channel, is expressed preferentially in peripheral sensory neurons and down-regulated after axotomy. Proc Natl Acad Sci USA 1998; 95:8963-8968. Identification and sequencing of a novel TTX-insensitive sodium channel in sensory neurons. It is generally thought that only one type of TTX-resistant sodium channels can be found in dorsal root ganglion cells. This is the first convincing description that there may be multiple TTX-resistant channels, and like the previously known SNS-sodium channel [36] it was shown that its mRNA is downregulated after axotomy. This observation has obvious implications for the development of novel analgesic drugs that target the sodium channel.

    • Distribution of the tetrodotoxin-resistant sodium channel PN3 in rat sensory neurons in normal and neuropathic conditions
    • Novakovic SD, Tzoumaka E, McGivern JG, Haraguchi M. Sangameswaran L, Gogas KR, et al. Distribution of the tetrodotoxin-resistant sodium channel PN3 in rat sensory neurons in normal and neuropathic conditions. J Neurosci 1998; 18:2174-2187. Researchers show that there is a redistribution of the classical tetrodotoxin-insensitive sodium channel SNS (also known as PN3) in sensory neurons after axotomy and that the downregulation of mRNA stands in striking contrast to the change of the channel protein in different locations of the damaged neuron. The paper argues that redistribution of ion channels is an important aspect of the generation of neuropathic pain. Yet unpublished reports from the same group show that this behaviour can contrast with the regulation and distribution of other sodium channels that are also found in sensory neurons.

This study demonstrated that PKCγ is localised to a small subset of sensory neurons in the substantia gelatinosa of the superficial dorsal horn of the spinal cord which receives synaptic input from small diameter nonpeptidergic sensory neurons with nociceptive function. Intriguingly, animals lacking this intracellular enzyme display a blunted pain response in models of neuropathic pain while retaining a normal sensitivity in models of noninflammatory pain.

A distinct subgroup of small DRG cells express GDNF receptor components and GDNF is protective for these neurons after nerve injury. J Neurosci 1998; 18:3059-3072. GDNF and NGF are capable of preventing the abnormal growth of Aß-fibres into the marginal layer and substantia gelatinosa of the spinal cord after axotomy. If this structural change in Aß-fibres causes abnormal sensations in painful neuropathies it raises the possibility that neurotrophic factors have a role for the prevention of the development of 'abnormal pain circuits' in the dorsal horn of the spinal cord.
