This study used punch biopsies of the skin to quantify the density of the epidermal innervation that is derived from unmyelinated afferents. It is shown that there is a reduction in the innervation density in painful neuropathies compared with normal control individuals.

This is the first convincing description that there may be multiple TTX-resistant channels, and like the previously known SNS-sodium channel [36] it was shown that its mRNA is downregulated after axotomy. This observation has obvious implications for the development of novel analgesic drugs that target the sodium channel.

Researchers show that there is a redistribution of the classical tetrodotoxin-insensitive sodium channel SNS (also known as PN3) in sensory neurons after axotomy and that the downregulation of mRNA stands in striking contrast to the change of the channel protein in different locations of the damaged neuron. The paper argues that redistribution of ion channels is an important aspect of the generation of neuropathic pain. Yet unpublished reports from the same group show that this behaviour can contrast with the regulation and distribution of other sodium channels that are also found in sensory neurons.

This study demonstrated that PKCγ is localised to a small subset of sensory neurons in the substantia gelatinosa of the superficial dorsal horn of the spinal cord which receives synaptic input from small diameter nonpeptidergic sensory neurons with nociceptive function. Intriguingly, animals lacking this intracellular enzyme display a blunted pain response in models of neuropathic pain while retaining a normal sensitivity in models of noninflammatory pain.
