The authors address possible strategies for managing patients infected and colonized with vancomycin-resistant S. aureus isolates. Although this is a 'theorectical' problem, in that no such clinical isolates have as yet been detected, this paper places the issue in appropriate perspective and demonstrates the magnitude of the problem that will be presented by these isolates if and when they are clinically identified.

Am J Epidemiol 1996, 143:496-504 Convincing evidence that the use of contact isolation in an MRSA epidemic was useful in limiting the nosocomial transmission of this organism is provided. The rate of transmission of MRSA from patients who were managed on contact isolation was significantly lower than the rate for patients who were not placed on isolation. in this study, the risk of nosocomial transmission of MRSA was reduced 16-fold by contact isolation during the outbreak. The results of this study, conducted in a neonatal intensive care unit, are consonant with the results of previous focused studies that demonstrated that contact isolation was effective in limiting the epidemic spread of MRSA.

Eur J Clin Microbiol Infect Dis 1996, 15:160-165. The authors evaluated the microbiological aspects of 52 consecutive patients undergoing surgical revision for presumably 'aseptic' joint loosening. More than 75% of the patients had positive cultures, and as one might predict, CNS were the predominant isolates. This manuscript provides yet another example of the fact that CNS isolates have become primary pathogens in infections of all types of medical prostheses.

Eur J Clin Microbiol Infect Dis 1996, 15:133-138. This is a retrospective review of the antimicrobial susceptibility of clinically significant blood isolates of Staphylococcus epidermidis at a tertiary care teaching hospital in Helsinki, Finland. The data are remarkable in that resistance to methicillin increased from 28 to 77% in the years from 1983-1994. These data are strikingly similar to data from the US Centers for Disease Control and Prevention's National Nosocomial Infections Study [129]. The rate of resistance among these clinically significant isolates nearly tripled in the years studied.

These four surgeons were all infected with a so-called 'pre-core mutant' of hepatitis B, which does not make hepatitis B 'e' antigen. The impact of these cases on the overall management of healthcare workers who are infected with HBV is unclear.

Using banked samples from an ongoing study of post-transfusion hepatitis. They demonstrated that hepatitis G virus infection was associated with only a mild hepatitis syndrome; all were anicteric with only mild elevations of alanine aminotransferase levels. In this cohort, 35 infections were identified among the 357 transfusion recipients; only three had hepatitis with hepatitis G as the sole viral marker. The authors noted that most of the hepatitis G virus infections were not associated with hepatitis and concluded that, at least thus far, no clear casual relation between hepatitis G virus and hepatitis could be established.
