This article reviews various pathophysiologic mechanisms contributing to vascular disease in coexistent hypertension and diabetes. Recent investigative data on abnormalities of platelet and vascular smooth muscle function, lipoprotein abnormalities, endothelial dysfunction, and the roles of insulin and insulin-like growth factor-1 in the pathogenesis of atherosclerosis are discussed extensively. The pathophysiology of diabetic nephropathy and new therapeutic guidelines are also addressed.

This prospective study described a strong positive correlation between BMI and the risk for diabetes mellitus in women. Modest weight gain in adult life was associated with an increased risk for diabetes mellitus. This association, though strongest in early adult life remained significant late into adulthood.

Hypertension 1996, 27:1180-1186. Insulin sensitivity in obesity hypertension was analyzed in obese hypertensives before and after weight loss. Enhanced insulin sensitivity after weight loss was associated with suppression of sympathetic nervous activity, decreases in blood pressure, and improvements in glucose metabolism.

Visceral adiposity is strongly associated with skeletal muscle insulin resistance. Potential mechanisms include increased availability of free fatty acids. This study demonstrated that this does not occur as a result of glucose-free fatty acid substrate competition, as was hypothesized earlier. In fact, women with visceral obesity were noted to have reduced rates of postabsorptive muscle free fatty acid utilization.

Data from this study demonstrated that systolic blood pressure was positively and independently associated with the ratio of dehydroepiandrosterone sulfate to free testosterone, and fasting insulin levels, but that diastolic blood pressure was associated with dehydroepiandrosterone sulfate to free testosterone ratio only, in a sample population of healthy young women. Serum androgen concentrations were suggested to be independent predictors of cardiovascular risk in women.

This prospective study concluded that a high plasma concentration in fasting individuals is associated with an increased risk for ischemic heart disease in nondiabetic men. Multiple logistic regression analysis revealed that this risk related to hyperinsulinemia remained significant after adjustments for systolic blood pressure, medications and family history of ischemic heart disease and was largely independent of the concomitant dyslipidemic state.

Circulation 1995, 91:1432-1443. These investigators carried out a cross-sectional study of the relation between arterial stiffness with impaired glucose tolerance and insulin concentrations. They reported that frank and borderline diabetic individuals have stiffer arteries, independent of artery wall thickness, compared with their euglycemic counterparts.

Describes severe insulin resistance with dyslipidemia and compensatory hyperinsulinemia in atheroembolic infarctions, but not in lacunar or cardioembolic infarctions, implicating these factors in the pathogenesis of large-cerebral artery atherosclerosis and atherothrombotic infarction. The study group comprised normotensive, nondiabetic, and nonobese patients.

This recent prospective study of Finnish children and adolescents showed a positive correlation between serum insulin and both systolic and diastolic blood pressures after standardizing for age and weight, suggesting that insulin appeared to regulate actual blood pressure within the normal range and also to predict future blood pressure, independent of age and weight.
