A subgroup analysis of the Scandinavian Simvastatin Survival Study (4S). Diabetes Care 1997, 20:614-620. This is the first placebo-controlled study which shows a significant reduction in major coronary events after lipid lowering therapy in NIDDM subjects. 202 NIDDM subjects from 35 to 70 years of age with coronary heart disease, hypercholesterolaemia and up to mild hypertriglyceridemia were treated with simvastatin 20-40mg daily for 5.4 years, and a 55% reduction in major coronary events was observed. The lowering of lipids or their probable correlation with the reduction of events were not given, leaving the basic mechanisms for improved prognosis speculative. The study population does not represent the NIDDM lipid profile in general.

N Engl J Med 1996, 335:1001-1009. This is the first placebo-controlled study to show that diabetic patients with coronary heart disease benefit from lipid lowering therapy started at least 3 months after myocardial infarction in spite of their lipid profile. 40 mg pravastatin daily for 5 years significantly reduced the risk of major coronary events by 24%, and the incidence curves start to differ between pravastatin and placebo groups after 2 years of treatment. The study group contains both insulin-dependent and NIDDM subjects from 21 to 75 years of age. The lipid lowering effect or its correlation with the reduction in major coronary events is not given.
