    • Canada-USA (CANUSA) Peritoneal Dialysis Study Group: Adequacy of dialysis and nutrition in continuous peritoneal dialysis: association with clinical outcomes. J Am Soc Nephrol 1996, 7:198-207. This is an important landmark paper on a multicentre, prospective cohort study assessing the predictive impact of total dose on outcome. It gives strong indicators of where the targets ought to be, but was not designed actually to set targets; increasing dialysis to compensate for the loss of residual renal function may not necessarily decrease the mortality rate, 75% of which is cardiovascular. The results should not be misinterpreted as having validated high, sometimes unachievable, small solute clearance targets and, in particular, it should not be viewed as having shown that CAPD is not an 'adequate' modality.

This is a 3-year prospective study in CAPD and haemodialysis patients, assessing outcomes (morbidity and survival) in relation to pretreatmant risk factors, adequacy, residual renal function and nutritional status. Patients were already on dialysis and were followed up for 3 years. Survival was linked to residual renal function, but not to normalized protein catabolic rate. The equivalence of KT/V on CAPD and haemodialysis was assessed.

In CAPD peritoneal transport determines urea clearance through an effect on ultrafiltration, so low transporters will have higher values for peritoneal dialysis KT/V. The limitations of a standard 4 × 2-litre regime in anuric, large and low transporter-status patients are highlighted.

The Watson formula might be the most practicable, but there are reservations.
