This study established the feasibility of using neural progenitors as vehicles for gene therapy for neurogenetic diseases. The report demonstrated that murine multipotent neural progenitors could engraft throughout the host brain and intermingle appropriately with endogenous cells, developing into integral members of multiple brain regions. The donor progenitors could be used to deliver sustained, therapeutic levels of a missing enzyme directly to and throughout the brain of a mouse model of a prototypical genetic neurodegenerative disease. The MPS VII mouse has lysosomal storage lesions disseminated throughout the brain which are refractory to other treatment approaches but which were cross-corrected by the widespread engraftment of enzyme-producing neural progenitors. The success of this approach was enabled by exploiting the inherent biologic properties of neural progenitors and stem cells, some of which were illuminated in this paper. For instance, delivering progenitors to germinal zones (e.g. the subventricular or ventricular zones) is one technique for assuring their efficient distribution appropriately throughout the CNS for disseminated disease. This work affirmed the linking of the two emerging fields of progenitor and stem cell biology with CNS gene therapy and repair. It also helped to lay the groundwork for a number of treatment strategies, such as replacement of degenerated cells, engineering cells to be resistant to toxins, delivery of missing metabolic products, over-expression of molecules, and substitution of alternate metabolic pathways.

This paper provided the unexpected and intriguing finding that impairments of spatial learning and memory in adult rats, induced by interruption of the major cholinergic input to the neocortex, could be reversed solely through the implantation of choline acetyl transferase-expressing fibroblasts into the denervated neocortical target regions. These results demonstrated that repletion of some substances (e.g. acetylcholine) by genetically engineered non-neuronal cells, even in a tonic nonsynaptically released fashion, in small amounts, to a relatively narrow region, may be sufficient for the restoration of some complex cognitive functions. This observation highlights the fact that the therapeutic needs, and the vehicles sufficient to achieve this, may vary from disease to disease.

Nerve growth factor-producing fibroblasts, implanted into the basal forebrain of aged rats, ameliorated their naturally occurring age-related memory impairments. However, the same cells implanted into normal young adult rats resulted in significant memory impairment.

Neurons of a particular type and particular location in adult rodent neocortex are targeted to degenerate by apopotic mechanisms, while other cell types and structures remain intact. This degeneration appears to create a microenvironment which may actually recapitulate certain embryonic developmental signals. This model provides an opportunity for studying the effects apoptosis might have in influencing the migration and differentiation of neural progenitors, perhaps to therapeutic ends. This possibility was explored in preliminary form in an abstract [72].
