Hypothesis: Compartmentalization of cytokines in intra-abdominal infection
-
Schein M, Wittmann DH, Holzheimer R, Condon RE: Hypothesis: Compartmentalization of cytokines in intra-abdominal infection. Surgery 1996, 119:694-700. Many patients with intra-abdominal infections die of uncontrolled sepsis despite clearance of the peritoneal cavity from infective concentrations of bacteria. The authors provide evidence that the relevant cytokine-mediated inflammatory process is compartmentalized in the peritoneal cavity, and suggest that novel therapeutic strategies should be directed against this local response.

Chir Gastroenteol 1995, 11(suppl 2):34-41. The authors discuss the failure of blocking the production or action of tumour necrosis factor and IL-1 in clinical sepsis trials concluding that anti-inflammatory therapy may only be successful in the initial hyperinflammatory phase of sepsis. They suggest exact immune analysis for a better stratification of patients for future studies.

Anderson ID, Fearon KCH, Grant IS: Laparotomy for abdominal sepsis in the critically ill. Br J Surg 1996, 83:535-539. Based on their experience with 125 patients with severe intra-abdominal sepsis, the authors discuss alternative surgical strategies in the treatment of these critically ill patients, concluding that aggressive surgical regimens may offer survival advantages.

Fisher CJ, Zheng Y: Potential strategies for inflammatory mediator manipulation:retrospect and prospect. World J Surg 1996, 20:447-453. This article reviews trials involving novel strategies of inflammatory mediator control in sepsis, concluding that so far they have failed to demonstrate clinical efficacy despite overwhelming animal data suggesting potential therapeutic benefit. The authors point out potential mistakes and the need for a more careful selection and stratification of patients, and standardization of therapy for future studies.

Pacelli F, Goglietto GB, Alftieri S, Piccioni E, Sgadari A, Gut D, Crucitti F: Prognosis of intra-abdominal infections. Arch Surg 1996, 131:641-645. This multivariate analysis of 604 patients with intra-abdominal infections shows a dominance of host-related factors over the type and source of infection on prognosis. Both, the APACHE-II Score and the Mannheim Peritonitis Index (as well as hypocholesterolaemia, and preoperative organ impairment) were independent predictors of death.

Reappraisal with meta-analysis of bacteremia, endotoxemia and mortality in gram negative sepsis
-
Hurley JC: Reappraisal with meta-analysis of bacteremia, endotoxemia and mortality in gram negative sepsis. J Clin Microbiol 1995, 33:1278-1282. This meta-analysis suggests that neither endotoxaemia nor Gram-negative bacteraemia alone can predict outcome in patients with suspected intra-abdominal sepsis. In combination, however, the two identified a subpopulation with a substantially increased risk for mortality.
