Many patients with intra-abdominal infections die of uncontrolled sepsis despite clearance of the peritoneal cavity from infective concentrations of bacteria. The authors provide evidence that the relevant cytokine-mediated inflammatory process is compartmentalized in the peritoneal cavity, and suggest that novel therapeutic strategies should be directed against this local response.
