This paper describes three cases of severe leptospirosis diagnosed at an inner-city hospital in Baltimore, Maryland, USA, and documents a new population at risk for acquiring the disease. PCR was able to diagnose the disease at initial presentation, and was used for establishing the epidemiology of the disease in the urban setting. This paper suggests that an endemic substrate for leptospirosis may exist where it has been little suspected, and that many more people may be infected by leptospires than have previously been documented.

Lancet 1996, 347: 535-536. This report details the laboratory investigation of the Nicaraguan outbreak of acute febrile illness and pulmonary hemorrhage. Leptospirosis was not suspected until autopsy studies revealed the presence of spirochetes in kidney tissue of people who died of pulmonary hemorrhage. Indirect immunofluoresence showed the spirochetes to be leptospires.
