Interstitial capillary leakage occurred within minutes after anaphylaxis onset and the mesenteric microcirculation showed strong macromolecular capillary leakage when compared with controls. The inflammation-induced microcirculatory changes with subsequent intravascular fluid transfer might be involved in the onset of the inaugural hypotension during anaphylactic shock.

They also suggest strategies for the management of anaphylaxis occurring during the third trimester of pregnancy, including the prompt administration of epinephrine and emergency cesarean delivery in cases of severe reactions.

The objective of this excellent retrospective study was to investigate how often epinephrine was used in the treatment of suspected perioperative anaphylaxis in Denmark and whether timing of treatment was important. The authors demonstrated that treatment with epinephrine was often delayed. A lack of a consistent response to suspected perioperative anaphylaxis was also documented. The authors conclude that it is imperative that anesthetic personnel be trained to diagnose and treat anaphylaxis in the difficult setting of surgery and anesthesia.
