Outcome of a procedure under Article 31 of Directive 2001/83/EC as amended. February, The European Medicines Agency has completed a review of the safety and effectiveness of pholcodine, following concerns that its use may put people at risk of developing anaphylaxis to NMBAs. The Agency’s Committee for Medicinal Products for Human Use has concluded that the existing evidence of the risk is weak, and that the benefits of pholcodine continue to outweigh its risks

Macromolecular capillary leakage is involved in the onset of anaphylactic hypotension
-
a rat model of anaphylactic shock, the authors investigated the microcirculatory permeability using a fluorescence confocal microscopy imaging system. Interstitial capillary leakage occurred within minutes after anaphylaxis onset and the mesenteric microcirculation showed strong macromolecular capillary leakage when compared with controls. The inflammation-induced microcirculatory changes with subsequent intravascular fluid transfer might be involved in the onset of the inaugural hypotension during anaphylactic shock

this review, the authors identify the clinical signs of anaphylaxis occurring during labor and cesarean delivery, discuss the more common allergens that cause anaphylaxis during this clinical setting, and develop a rational approach to the identification of the offending allergen. They also suggest strategies for the management of anaphylaxis occurring during the third trimester of pregnancy, including the prompt administration of epinephrine and emergency cesarean delivery in cases of severe reactions

Quantification of volume loss and haemodynamic changes of Gelofusine-induced anaphylaxis during cardiopulmonary bypass
-
An excellent case report of gelatin-induced anaphylaxis. The patient required 51 ml/kg of resuscitative fluids in the 15 min after onset of anaphylaxis, corresponding to 73% of her calculated preoperative blood volume. The authors emphasize the importance of volume resuscitation in the treatment of anaphylaxis

Treatment with epinephrine (Adrenaline) in suspected anaphylaxis during anesthesia in Denmark
-
The objective of this excellent retrospective study was to investigate how often epinephrine was used in the treatment of suspected perioperative anaphylaxis in Denmark and whether timing of treatment was important. The authors demonstrated that treatment with epinephrine was often delayed. A lack of a consistent response to suspected perioperative anaphylaxis was also documented. The authors conclude that it is imperative that anesthetic personnel be trained to diagnose and treat anaphylaxis in the difficult setting of surgery and anesthesia
