National Kidney Disease Education Program. http://www.nkdep.nih.gov/professionals/CKD-DrugDosing-508. pdf. [Accessed 3 March 2011] This document provides the rationale of the NKDEP for recommending use of either the Cockcroft-Gault or MDRD equation for determining drug dosing. Importantly, the NKDEP recommends converting the MDRD equation to units of ml/min when used for drug dosing in patients with very large or small BSAs and the use of measures of GFR or ClCr in patients in whom the estimating equations would not be expected to be accurate.
