This brief report demonstrates a successful example of how the optimal waist circumference cut-off for the diagnosis of the metabolic syndrome may be derived from a nationally representative survey. It also includes references to details of how such a survey can be designed and conducted. This article has established the optimal waist cut-off for Iran at 90 cm for both men and women.
