artery occlusion. Am J Ophthalmol 2008; 146:700-706. Intravenous thrombolytic therapy was administered to 17 eyes of patients with acute CRAO (<12 h). Overall, good visual outcome strongly correlated with time to treatment. Seven of 17 (41 %) eyes treated within 6.5 h had a visual acuity of 20/50 or better. In comparison, none of the 11 patients treated after 6.5 h from the onset of symptoms achieved a visual acuity of 20/50 or better. Of those patients who received i.v. thrombolytic therapy within 6.5 h, nine (53%) had three lines of improvement in vision. In comparison, no patient treated after 6.5 h had a similar improvement in vision.

This nonrandomized, single-center study evaluated the benefits of administering 3mg aliquots of intraarterial tPA in 42 patients compared with 21 patients who received conventional therapy. Seventy-six percent of patients who received intraarterial thrombolytic therapy had at least one line of visual acuity improvement compared with only 33% of patients who received conventional therapy. In addition, 33% of patients in the intraarterial thrombolytic therapy group experienced an improvement of three lines or more of vision compared with only 4.8% of patients in the conventional therapy group
