This paper shows that co-infection with HIV results in 10 times greater MDR tuberculosis. Prior treatment or prophylaxis did not increase rates of multidrug resistance.

This exciting paper investigates community-based treatment for MDR tuberculosis and finds very encouraging results, even in a resource poor setting. Early initiation of appropriate therapy was key.

This important case series identifies high rates of mental illness in patients with MDR tuberculosis. The development of new psychiatric illness on treatment is also discussed. Therapy options are discussed, and it is concluded that treatment of the mental illness need not compromise anti-tuberculosis therapy.

It recommends culture and drug-sensitivity testing in all patients who remain sputum smear positive at 2 months.

This interesting case series highlights the dangers of neglecting pyridoxine prophylaxis during treatment for MDR tuberculosis. Treatment strategies for neuropathy in these cases are outlined.

70. Olender S, Saito M, Apgar J, et al. Low prevalence and increased household clustering of Mycobacterium tuberculosis infection in high altitude villages in Peru. Am J Trap Med Hyg 2003; 68:721-727. This elegant prospective study showed reduced tuberculosis skin test positivity at greater altitude despite increased household clustering. Potential mechanisms are succinctly discussed.
