JAMA 2007, 297:611-619. An excellent and lucid study demonstrating the role of state-of-theart assessment of risk, using a combination of clinical historical factors and biomarkers, including hsCRP, to determine future risk of cardiovascular events among women. The study reports the latest clinical epidemiologic methods and uses global vascular risk as an outcome, rather than more limited outcomes related to heart disease only.

Stroke 2008, 39:1456-1463. One of the major strengths of this study is its focus on a particular etiologic subtype of stroke, intracranial atherosclerosis. This is an important advance because stroke, unlike heart disease, is heterogeneous and many stroke studies do not distinguish among stroke subtypes.

Stroke 2006, 37:482-486. In this large study using multimodal biomarker analysis, including infectious and inflammatory serum biomarkers and Doppler, inflammatory biomarkers predicted stoke risk in patients with asymptomatic carotid stenosis. This provides supportive evidence that inflammation may be used to distinguish low- and high-risk carotid patients. This provides an important direction for new research, and if confirmed, these findings could lead to new approaches to risk stratification in such patients. (Pubitemid 43732010)

Circulation 2006, 114:1549-1553. This guideline from two major cardiology societies recommends using influenza vaccination to prevent recurrent cardiovascular events among patients with existent cardiovascular disease. This represents the first anti-infective strategy widely used to prevent vascular disease. Stroke patients were not explicitly included in the guidelines, but it may be reasonable to extrapolate these recommendations to stroke patients as well. (Pubitemid 44527011)
