Meta analysis of studies of early risk of stroke following TIA. All studies reporting the risk of stroke at 2 and 7 days after TIA were identified using multiple methods of electronic and hand searching. Rate of stroke following TIA at 7 days was 5.2% among 10126 patients. Significant heterogeneity between studies was best explained by study setting and method.

Redgrave JN, Coutts SB, Schulz UG, et al. Systematic review of associations between the presence of acute ischemic lesions on diffusion-weighted imaging and clinical predictors of early stroke risk after transient ischemic attack. Stroke 2007; 38:1482-1488. Systematic review of associations between DWI abnormalities and clinical predictors of stroke afterTIA. Nineteen relevant studies were identified using electronic and hand searches. Determinants of DWI positivity following TIA were studied. Longer symptom duration, motor weakness, dysphasia and dysarthria were associated with the presence of lesions on DWI, whereas sex, hypertension and diabetes were not.

Kennedy J, Hill MD, Ryckborst KJ, et al., for the FASTER Investigators. Fast assessment of stroke and transient ischaemic attack to prevent early recurrence (FASTER): a randomised controlled pilot trial. Lancet Neurol 2007; 6:961-969. Randomized controlled trial of clopidogrel and aspirin versus aspirin alone and simvastatin versus placebo for stroke prevention in 392 patients in the acute phase after TIA. The primary outcome was stroke at 90 days. Fourteen (7.1 %) patients on clopidogrel had a stroke within 90 days compared with 21 (10.8%) patients on placebo [risk ratio 0.7 (95% Cl 0.3-1.2)]. Twenty-one (10.6%) patients on simvastatin had a stroke within 90 days compared with 14 (7.3%) patients on placebo [risk ratio 1.3 (0.7-2.4)].
