7. Mastrangelo D, De Francesco S, Di Leonardo A, et al. Does the evidence matter in medicine? The retinoblastoma paradigm. Int J Cancer 2007; 121:2501-2505. This is a provocative study challenging the nearly 40-year-old Knudson's 'two-hit' theory of Rb development using epidemiologic evidence to support its claims. Larger patient sample sizes are needed to support these authors' claims, but the article is extremely important and its conclusions will undoubtedly be explored by other centers in the future.

9. Dimaras H, Khetan V, Halliday W, et al. Loss of RB1 induces nonproliferative retinoma: increasing genomic instability correlates with progression to retinoblastoma. Hum Mol Genet 2008; 17:1363-1372. This study is an intriguing examination of the theory that retinomas are a benign precursor lesion for Rb and is the first article to explore the differences in the expression of various tumor suppressor genes and oncogenes between retinoma tissue and Rb tissue.
10. Chakraborty S, Khare S, Dorairaj SK, et al. Identification of genes associated with tumorigenesis of retinoblastoma by microarray analysis. Genomics 2007; 90:344-353. This is the first study in which large-scale identification of candidate genes for Rb development/progression was performed using microarray analysis.

This study is highlighted by the highest ocular salvage and EBR avoidance rates for Reese-Ellsworth Group Vb disease using chemotherapy and focal therapy reported to date.
13. Carcaboso AM, Bramuglia GF, Chantada GL, et al. Topotecan vitreous levels after periocular or intravenous delivery in rabbits: an alternative for retinoblastoma chemotherapy. Invest Ophthalmol Vis Sci 2007; 48:3761-3767. This is the first study examining the pharmacologie penetration of topotecan into the vitreous cavity of animals. This study is important because this agent has promise as an alternative drug to be used for Rb patients, and its results have resulted in a clinical trial in human Rb patients.
14. Tsui JY, Dalgard C, Van Quill KR, et al. Subconjunctival topotecan in fibrin sealant in the treatment of transgenic murine retinoblastoma. Invest Ophthalmol Vis Sci 2008; 49:490-496. This study continues previously published work on the use of a fibrin sealant to extend the duration of time of transscleral penetration of a drug. The work is important, as rapid drop-off in vitreous concentration of chemotherapeutic agents after subconjunctival administration is an impediment to efficacy.
15. Jockovich ME, Suarez F, Alegret A, et al. Mechanism of retinoblastoma tumor cell death after focal chemotherapy, radiation, and vascular targeting therapy in a mouse model. Invest Ophthalmol Vis Sci 2007; 48:5371-5376. This is an important work identifying the main mechanism of action of cell death to be apoptosis, rather than necrosis, after traditional and modern approaches to the treatment of murine Rb.
16. Boutrid H, Jockovich ME, Murray T, et al. Targeting hypoxia, a novel treatment for advanced retinoblastoma. Invest Ophthalmol Vis Sci 2008; 49:2799-2805. This is a cutting-edge work examining the use of an entirely new treatment modality, glycolytic inhibitors for the treatment of murine Rb. This type of drug may be eventually an important adjunctive treatment for targeting tumor cells resistant to chemotherapy in the care of human Rb.

Pediatr Blood Cancer 2008; 50:692-694. This study addresses an important controversy in the treatment of high-risk Rb after enucleation, outlining the variability of care around the world. 22 Abramson DH, Dunkel U, Brodie SE, et al. A phase I/II study of direct intraarterial (ophthalmic artery) chemotherapy with melphalan for intraocular retinoblastoma initial results. Ophthalmology 2008; 115:1398-1404,1404.e1. This is the most important clinical study of the last year; the authors report the administration of intraarterial chemotherapy administered by cannulation of the ophthalmic artery. Fundus findings after treatment were dramatic, and no retinal toxicity was observed.

This is the first study exploring the behavior patterns of Rb survivors regarding routine self-examination patterns or imaging studies or both they routinely obtain given the knowledge of their diagnosis.
