Sacco RL, Ellenberg JH, Mohr JR Tatemichi TK, Hier DB, Price TR, Wolf PA. Infarcts of undetermined cause: the NINCDS Stroke Data Bank. Ann Neurol 1989; 25:382-390. This article reports the prevalence and characteristics of infarcts of undetermined cause in a large multi-center registry. Although evaluation did not involve magnetic resonance imaging or transesophageal echocardiography, the large proportion of strokes of uncertain etiology, 40%, highlights the scope of cryptogenic embolism in the etiology of brain ischemia.

Kerr AJ, Buck T, Chia K, Chow CM, Fox E, Levine RA, Picard MH. Transmitral Doppler: a new transthoracic contrast method for patent foramen ovale detection and quantification. J Am Coll Cardiol 2000; 36:1959-1966. This study describes a new contrast technique for the detection of right-to-left shunts by transthoracic echocardiography using transmitral Doppler imaging. The diagnostic accuracy of this method proved to be comparable to transesophageal echocardiography, but the results require independent confirmation.
