Newby D. Efficacy of noninvasive ventilation in patients with acute cardiogenic pulmonary oedema: the 3CPO trial. Hot line session. Clinical Trial Update. ESC Congress 2007, Vienna. This study was presented in the last European Congress of Cardiology and has not been published yet. More than 1000 patients were assigned into one of the three groups: CPAP, NIPSV or conventional oxygen therapy. It is the largest trial on noninvasive ventilation carried out to date. Both NIV techniques improved the respiratory failure faster but mortality was not different.

This randomized trial in patients with APE showed that CPAP or bilevel NIPSV had similar effects. No differences in the myocardial infarction rate and the primary endpoint were seen

Smith LA, Vennelle M, Gardner RS, et al. Auto-titrating continuous positive airway pressure therapy in patients with chronic heart failure and obstructive sleep apnoea: a randomized placebo-controlled trial. Eur Heart J 2007; 28:1221-1227. This study showed an improvement in OSA with the use of CPAP, without impact on heart failure parameters. These results differed slightly from those of the CANPAP trial that demonstrated an increase in left ventricular ejection fraction with the use of CPAP.

Suppression of central sleep apnea by continuous positive airway pressure and transplant-free survival in heart failure: a post hoc analysis of the Canadian Continuous Positive Airway Pressure for Patients with Central Sleep Apnea and Heart Failure Trial (CANPAP). Circulation 2007; 115:3173-3180. This is a posthoc analysis of the CANPAP trial. In the original paper, CPAP improved several respiratory and cardiac parameters, with an increase in the LVEF, but these effects did not reduce mortality. However, in this trial transplant-free survival was higher in the CPAP group.
