The authors give a clinically oriented overview of different aspects of the disease, including the role of urinary antigen testing and PCR for the diagnosis of S. pneumoniae infection, the use of therapies designed to modulate host-related immune responses, the importance of effective triage, and the deleterious effects of deviation from protocols.

An opportune review because Legionella spp. have progressively become recognized as relatively common causative agents of both community-acquired and nosocomial legionellosis.
3. Oosterheert JJ, Bonten MJ, Hak E, Schneider MM: Severe community pneumonia: what's in a name? Curr Opin Infect Dis 2003, 16:153-159. A comprehensive review of different definitions of severe community-acquired pneumonia and whether a differentiation based on these definitions reflects variation in etiology, risk factors, diagnostic approaches, and treatment. The authors emphasize the importance of clinical judgment as current guidelines do not adequately identify patients with high risk of mortality and intensive care unit admittance.

A systematic review. J Antimicrob Chemother 2003, 52:553-563. The results of this interesting review do not support that treatment with a β-lactam plus macrolide or quinolone monotherapy is truly superior to β-lactam treatment alone for patients hospitalized with community-acquired pneumonia.

The predictive rules were validated against the pneumonia severity index (PSI) scoring system.

This analysis of two previously reported studies demonstrates the efficacy of the once-a-day parenteral β-lactam agent, ertapenem, for the treatment of hospitalized patients with moderate-to-severe community-acquired pneumonia.

This paper presents a pooled data analysis of two previous studies and shows that sequential intravenous/oral moxifloxacin was as effective as other fluoroquinolones and a β-lactam/macrolide combination for treating hospitalized patients with severe community-acquired pneumonia.
