The study reports on the evaluation and performance of several dozen response measures in the context of their retrospective assessment in clinical trials and correlation with rheumatologists' opinion on improvement. The resulting hybrid ACR measure performed well in these analyses but is quite complex. It will have to be tested prospectively in future clinical trials against other less complex measures.

Arthritis Rheum 2007; 57:440-447. In this study, different definitions of remission and low (or minimal) disease activity are used in a population of patients treated with nonbiological DMARDs. The authors reveal the differences in the proportions of patients attaining the respective states and point to the importance of clearly defining the instruments used in clinical trials. They also reveal the value of achieving low disease activity or remission with respect to good functional and radiographic outcomes.
    • Khanna D, Oh M, Furst DE, et al. Evaluation of the preliminary definitions of minimal disease activity and remission in an early seropositive rheumatoid arthritis cohort.

The BeSt trial has become a classic strategic trial employing treatment decisions upon tight control, which has significantly influenced current thinking. The two year data are presented here. While, as a consequence of the therapeutic strategies employed which involved biological agents at some time point in all populations, the different groups have achieved similar results at endpoint, patients who were started on intensive therapies fare particularly well.

Arthritis Rheum 2007; 56:3226-3235. Ttreatment response at 3-6 months from start of therapy is highly predictive of the response at endpoint (1 year), irrespective of the type of therapy. This indicates that therapies that are ineffective in terms of reaching low disease activity within 3-6 months should be discontinued and other treatment options employed.
