Arthritis Rheum 2006; 54:628-634. This large observational study of patients with RA in the National Databank for Rheumatic Disease cohort. Glucocorticoid use (mean dose 5 mg/kg) was associated with an increased risk of hospitalization for pneumonia and this association showed a dose-related increase in risk. Interestingly the risk associated with antitumor necrosis factor therapies was much less than glucocorticoids despite theoretical concerns about increased risk of infection.

This cross-sectional Italian study found increased incidence of asymptomatic vertebral fracture in patients with a variety of underlying diseases requiring chronic glucocorticoid use (>1350 mg cumulative dose equivalent). This study cannot separate disease-specific outcomes from glucocorticoid-associated ones. As seen in other studies low BMD did not correlate with fracture occurrence.

No significant mood changes were found, although a significant decrease in aspects of memory performance was observed after the first prednisone exposure.

This was a double-blind randomized placebo-controlled trial of patients with acute respiratory distress syndrome who received methylprednisilone or placebo. Methylprednisilone was associated with higher rates of mortality. Compared with placebo, methylprednisolone did not increase the rate of infectious complications but was associated with a higher rate of neuromuscular weakness. The increased rate of neuromyopathy was not seen on further retrospective chart review.

This is a retrospective chart review of boys with dystrophinopathies treated with steroids for at least 1 year compared with boys who had never been treated with steroids. Vertebral compression fractures occurred in 32% of the treated group, whereas no vertebral fractures were discovered in the steroid-naive group. Long bone fractures were 2.6 times greater in steroid-treated patients.
