Nonmyeloablative preparative regimens have been reported to be associated with late GHVD. As IFIs are highly associated with GVHD, these data help inform clinical assessments of risk and the design of antifungal protocols. More importantly, by understanding how the preparative regimen influences the nature of immune reconstitution, specific risk factors for IFI may be uncovered.

The enhanced reconstitution of functional antifungal immune response is not major histocompatibility complex-restricted. These observations expand the potential utility of enriching the allograft with myeloid progenitors, bridging chemotherapy-induced neutropenia, or infusions in genetic or acquired disorders of neutrophil deficiency.

    • Vaccine immunity to pathogenic fungi overcomes the requirement for CD4 help in exogenous antigen presentation to CD8+ T cells: Implications for vaccine development in immune-deficient hosts
    • Wuthrich M, Filutowicz HI, Warner T, et al. Vaccine immunity to pathogenic fungi overcomes the requirement for CD4 help in exogenous antigen presentation to CD8+ T cells: implications for vaccine development in immune-deficient hosts. J Exp Med 2003; 197:1405-1416. The plasticity of the nonmyeloid response is demonstrated in this model of murine fungal infection. Antigens of Blastomyces dermatrtidis or Histoplasma capsulatum were able to activate memory CD8+ cells in a major histocompatibility complex class-I manner even in the absence of CD4+ cells. CD8+ T cell-derived cytokines tumor necrosis factor-α, Interferon-γ, and granulocyte/macrophage colony-stimulating factor mediated durable vaccine immunity. Thus, a vaccine approach may be useful, even in a state of depleted lymphocyte populations that exists following HCT.

Infliximab use in patients with severe graft-versus-host disease and other emerging risk factors of non-Candida invasive fungal infections in allogeneic hematopoietic stem cell transplant recipients: a cohort study. Blood 2003; 102:2768-2776. The anti-tumor necrosis factor-α antibody, Infliximab, was associated with a significant increase in IFI in patients with severe GVHD. This observation confirms that nonmyeloid responses are clinically significant with respect to the control of invasive rungal infection.
