Instead, the usual treatments of decoherence suffer from the same internal inconsistency that constitutes the infamous "measurement" problem of quantum foundations, as discussed by Wigner and Mittelstaedt among others. Solving the measurement problem requires that one introduce probabilities in quantum theory in a consistent manner, as discussed in, and then decoherence can be consistently discussed as a quantum mechanical process: see [R.
