Benedetti F, Arduino C, Costa S, et al. Loss of expectation-related mechanisms in Alzheimer's disease makes analgesic therapies less effective. Pain 2006; 121:133-144. In this both scientifically and clinically very important study, the authors assessed the interaction of verum and placebo mechanisms in patients with Alzheimer's disease, and found impaired placebo responses with loss of frontal lobe function (frontal assessment battery scores) and connectivity to other brain areas. This study indicates that loss of frontal lobe-mediated expectancy mechanisms, as in Alzheimer's disease, disrupt placebo analgesia and thus makes analgesic therapies less effective.

Keltner JR, Furst A, Fan C, et al. Isolating the modulatory effect of expectation on pain transmission: a functional magnetic resonance imaging study. J Neurosci 2006; 26:4437-4443. A very elegant study that addresses the question of how expectancy modulates pain perception and to what extent such modulation is accompanied by activation in brain areas implicated in descending modulatory processes. This study complements previous studies of pain expectancy by showing that pain intensity expectancy acts via a modulatory network that converges with afferent nociceptive input in the nucleus cuneiformis.

Blood oxygenation level-dependent visualization of synaptic relay stations of sensory pathways along the neuroaxis in response to graded sensory stimulation of a limb. J Neurosci 2006; 26:6330-6336. This is a methodologically groundbreaking study with respect to the research of pain. Lilja et al. have successfully conducted fMRI of the spinal cord and medulla of the rat in response to electrical stimuli and were able to functionally dissociate the spinothalamic tract and the dorsal column pathway depending on the stimulus intensity used. In addition, the authors convincingly demonstrate a neuropharmacological modulation of these signals.

Song GH, Venkatraman V, Ho KY, et al. Cortical effects of anticipation and endogenous modulation of visceral pain assessed by functional brain MRI in irritable bowel syndrome patients and healthy controls. Pain 2006; 126:79-90. This study addresses the question of whether a dysbalance in pro and antinociceptive mechanisms may underlie the pathogenesis of irritable bowel syndrome. The authors demonstrate that behavioral differences as a result of rectal stimulation between patients and controls are paralleled by differences in pain-related central responses, which can be attributed to the endogenous pain modulatory system.

Becerra L, Harter K, Gonzalez RG, Borsook D. Functional magnetic resonance imaging measures of the effects of morphine on central nervous system circuitry in opioid-naive healthy volunteers. Anesth Analg 2006; 103:208-216. This is an extensive review article reviewing the contribution of fMRI to the understanding of acute and chronic pain, the endogenous pain control systems and the evaluation of analgesic substances. Besides the comprehensive discussion of the data, key findings related to each topic are summarized in clearly laid out tables.

Schweinhardt P, Bountra C, Tracey I. Pharmacological FMRI in the development of new analgesic compounds. NMR Biomed 2006; 19:702-711. This is an excellent review on the recent advances in the development of pharmacological imaging. It provides a comprehensive background of the potential contribution of pharmacological imaging to pain research, outlines important methodological considerations that have to be consideredwhen performing phFMRI, and summarizes the results of pharmacological imaging studies available so far.
