Familial sinus bradycardia associated with a mutation in the cardiac pacemaker channel
-
In a large Italian family with asymptomatic bradycardia, the authors found that the bradycardic phenotype is tightly associated with a single-point mutation of the HCN4 pacemaker channel. Mutated channels activate at more negative voltages than do wild-type channels, which explains the heart rate-slowing effect of the mutation. This is the first report of a HCN channel mutation specifically affecting heart rate.

The authors compared ivabradine and atenolol in 939 patients with stable chronic angina in a double-blind non-inferiority trial, with the aim of comparing anti-anginal and anti-ischemic actions of ivabradine and the β -blocker atenolol. Non-inferiority of ivabradine was found for all exercise parameters.

Properties of ivabradine-induced block of HCN1 and HCN4 pacemaker channels
-
The authors compared the blocking actions of ivabradine on HCN1 and HCN4 isoforms, and found that, although the drug acts as an 'open channel' blocker with HCN4, similar to native 'funny' channels, it acts as a 'closed channel' blocker with HCN1. This finding highlights the possibility of further exploiting these blocking differences with new drugs to accentuate HCN-isoform specificity.

Bucchi A., Tognati A., Milanesi R., Baruscotti M., and DiFrancesco D. Properties of ivabradine-induced block of HCN1 and HCN4 pacemaker channels. J Physiol 572 (2006) 335-346.
