Randomized effectiveness trial of a computer-assisted intervention found significant improvements in both the number of laboratory assays and the patient-centered aspects of diabetes care compared with those in the control group. Overall improvements in HbA1c, quality of life and depression scores were observed.

Qualitative study analyzed the implementation activities of intervention organizations as part of a larger effectiveness evaluation of yearlong collaboratives. Sites averaged 30 different change efforts each to implement the Chronic Care Model. Fidelity to the model was high.

Diabetes Care 2006; 28:811-817. This study was a randomized-controlled trial that tested the effectiveness of implementing the entire Chronic Care Model into an underserved community. Marked declines in HbA1c and non-HDL cholesterol were observed. The proportion of subjects self-monitoring their blood glucose increased significantly following the intervention.

A randomized trial of a primary care-based disease management program to improve cardiovascular risk factors and glycated hemoglobin levels in patients with diabetes. Am J Med 2005; 118:276-284. Randomized-controlled trial found that subjects in a pharmacist-led, primary-care-based disease management program had significantly greater improvements in systolic blood pressure and HbA1c than the control group. The proportion of subjects using Aspirin therapy was also significantly higher in the intervention group.
