Annual incidence of Churg-Strauss syndrome among asthma patients was reported to be 64.4 per million patients by Martin et al. [2]. In this retrospective study conducted on 184 667 patients who had been prescribed ≥3 asthma drugs in three US centers between 1994 and 2000, 21 were classified as having 'probable/definite' Churg-Strauss syndrome. Therefore, overall annual incidence of Churg-Strauss syndrome among asthma patients, 34.6 per million patients, may be somewhat lower than previously reported, and with no significant variations in the studied population between 0 and 97 for sex, age group or period.

In this Italian cohort of 93 patients with Churg-Strauss syndrome, 35 (37.6%) were ANCA-positive, yielding a perinuclear-labeling pattern for 74.3% of them, with MPO specificity for 92%. ANCA positivity was associated with higher frequencies of renal disease and alveolar capillaritis, but with lower rates of lung disease (other than alveolar hemorrhage) and cardiac involvement.

This paper reports the results of the analysis of the 112 patients with Churg-Strauss syndrome enrolled in the trials conducted by the French Vasculitis Study Group between December 1995 and December 2002. ANCA were detected at diagnosis in 43 (38%) patients, who had more frequent renal involvement, peripheral neuropathy and biopsy-proven vasculitis, whereas ANCA-negative patients were more likely to have heart disease and fever. Results of the analyses support the notion that ANCA-positive and ANCA-negative patients with Churg-Strauss syndrome might differ phenotypically, with the former being more characteristic of a predominantly vasculitic pattern.

Serum vascular endothelial growth factor levels were measured in 18 patients with Churg-Strauss syndrome, 19 patients with asthma and 12 patients with acute bronchitis. Levels were significantly higher in patients with Churg-Strauss syndrome and decreased after successful therapy. The sensitivity and specificity of distinguishing Churg-Strauss syndrome from asthma were 93.3% and 81.8%, respectively, suggesting that vascular endothelial growth factor might be associated with the pathogenesis of Churg-Strauss syndrome and could be a useful marker of the disease, and maybe a target for future therapy.
