Compared with its delivery in oxygen, adding sevoflurane at the start of 66% nitrous oxide delivery, when the uptake of nitrous oxide would be great, and after an hour of nitrous oxide administration, when the uptake of the latter would be minimal, resulted in an identical reduction in the alveolar-inspired sevoflurane difference. This questions the belief that the second gas effect only operates during large volume uptake and suggests that changes in ventilation-perfusion inhomogeneity contribute to the mechanism.
