An interesting study using the lowest reported dose of bupivacaine (2.5 mg) combined with fentanyl to produce a selective 'saddle block'. The study also included a TIVA group, which allowed earlier discharge home. On the contrary, bupivacaine 5mg alone prolonged the time to home-readiness significantly compared with the combination group.

In this study an adequate duration and density of spinal anaesthesia was achieved when using 40 mg of 2-CP with a significantly faster ability to walk and void compared to bupivacaine. However, a relatively large dose of bupivacaine (7.5 mg) without unilateral spinal anaesthesia was used, which might explain both the delayed recovery and the variability in motor block with bupivacaine.

This meta-analysis consisting of >1000 patients with central neuraxial block showed better pain relief and a decreased need for postoperative opioids after regional anaesthesia compared with general anaesthesia. Despite of this better early recovery, the ambulatory surgery unit stay was not shorter after regional anaesthesia.
