It suggests that a prospective randomized trial is needed.

This study showed very promising early results in terms of a reduction in acute rejection and possibly a reduction in the rate of decline in the forced expiratory volume in 1 s. The benefit was less at 24 months, but highlights problems with withdrawal as a result of toxicity of the study drug.

Conversion to sirolimus and mycophenolate can attenuate the progression of bronchiolitis obliterans syndrome and improves renal function after lung transplantation
-
Groetzner J, Wittwer T, Kaczmarek I, et al. Conversion to sirolimus and mycophenolate can attenuate the progression of bronchiolitis obliterans syndrome and improves renal function after lung transplantation. Transplantation 2006; 81:355-360. This prospective study showed stabilization of graft function after a diagnosis of chronic rejection, and suggests the need for early intervention.

A randomized trial of inhaled cyclosporine in lung-transplant recipients
-
Iacono AT, Johnson BA, Grgurich WF, et al. A randomized trial of inhaled cyclosporine in lung-transplant recipients. N Engl J Med 2006; 354:141-150. This prospective study suggests that this novel route of drug administration is safe and tolerable, and although it did not reduce rates of acute rejection suggested improved survival.
