This review article provides an excellent background on the pathophysiology and treatment of cardiogenic shock with references to many of the studies that occurred between 1995 and 2004.
2. The CREATE-ECLA Trial Group. Effect of glucose-insulin-potassium infusion on mortality in patients with acute ST-segment elevation myocardial infarction: the CREATE-ECLA Randomized Controlled Trial. JAMA 2005; 293:437-446. This article is particularly important because it addresses current therapy in countries other than Europe and the USA. It clearly demonstrates that glucose/insulin/potassium infusion does not affect mortality following STEMI.
3. Babaev A, Frederick PD, Pasta DJ, et al. Trends in management and outcomes of patients with acute myocardial infarction complicated by cardiogenic shock. JAMA 2005; 294:448-454. The National Registry of Myocardial Infarction investigators examined the current management patterns and in-hospital mortality of AMI complicated by cardiogenic shock. They provide evidence that guidelines are slow to be adopted into practice.

5. Jeger RV, Harkness SM, Ramanathan K, et al. Emergency revascularization in patients with cardiogenic shock on admission: a report from the SHOCK trial and registry. Eur Heart J 2006; 27:664-670. Documents the worse outcome associated with cardiogenic shock on admission but also demonstrates that aggressive therapy is important in this group of patients.
