This prospective double-blind, controlled study evaluates the use of guar gum, a soluble fibre in 20 enterally fed patients with persistent diarrhoea. The use of soluble fibre correlated with decreased diarrhoeal episodes.

This prospective controlled study investigated the progression of enteral nutrition in two groups of patients. Caloric prescription was 5 kcal/kg body weight higher in the treatment group (group 2) compared with standard (group 1). Group 2 showed faster progressions of calorie and protein intakes than group 1. This trial was, however, neither randomized nor blinded and group 2 was recruited 2 years after group 1. We therefore need to view these results with great caution.

ICUs adhering to the clinical practice guidelines were more likely to feed patients successfully via the enteral route.

Evidence-based recommendations were introduced in intervention hospitals. This trial could show that patients in the intervention hospitals received significantly more days of enteral nutrition and had a shorter mean stay in hospital. A trend towards reduced mortality was observed in the intervention group.

In this RCT, the authors investigated a possible correlation between the amount of residual volume and the risk of aspiration in patients who were either fed with nasogastric or PEG tube. The authors found no correlation between the amount of residual volume and the risk of regurgitation or aspiration. They concluded it was a poor marker for the risk of aspiration. Interestingly, the frequency of regurgitations was significantly reduced with PEG tubes compared with nasogastric tubes.

Lancet 2005; 365:764-772. Two large multicentre RCTs of stroke patients with dysphagia comparing early nasogastric with no nutrition and nasogastric with PEG feeding. The primary outcome was death or poor outcome. Early nasogastric feeding compared with no nutrition was associated with a reduction in death, PEG feeding was associated with an increased risk of death.
