Does magnetoencephalography add to scalp video-EEG as a diagnostic tool in epilepsy surgery?
-
Pataraia E, Simos PG, Castillo EM, et al. Does magnetoencephalography add to scalp video-EEG as a diagnostic tool in epilepsy surgery? Neurology 2004; 62:943-948. MEG could localize the resected region in 72.3% of patients, noninvasive video EEG only in 40%. MEG contributed to the localization of the resected region in 58.8% of patients with a nonlocalizing video EEG study and 72.8% of patients for whom video EEG only partially identified the resected zone. MEG and video EEG results were equivalent in 32.3% of cases, and additional localization information was obtained using MEG in 40% of patients.

Papanicolaou AC, Pataraia E, Billingsley-Marshall R, et al. Toward the substitution of invasive electroencephalography in epilepsy surgery. J Clin Neurophysiol 2005; 22:231-237. Invasive video EEG and MEG localization results were comparable and localized the epileptogenic zone correctly in 54% and 56% of patients, respectively. MEG seemed to have a slight but nonsignificant advantage over invasive video EEG for temporal cases, whereas the reverse was true for extratemporal cases.

-
Papanicolaou AC, Simos PG, Castillo EM, et al. Magnetoencephalography: a noninvasive alternative to the Wada procedure. J Neurosurg 2004; 100:867-876. MEG and Wada data showed a high degree of concordance (87%). MEG laterality judgments had an overall sensitivity of 98%, but a lower selectivity of 83%, which was due to the fact that MEG detected more activity in the nondominant hemisphere than was predicted based on the Wada test.

Breier JI, Castillo EM, Simos PG, et al. Atypical language representation in patients with chronic seizure disorder and achievement deficits with magnetoencephalography. Epilepsia 2005; 46:540-548. Reading and spelling achievement deficits in patients with complex partial seizures of left-hemisphere origin are associated with atypical language organization.

Organization of receptive language-specific cortex before and after left temporal lobectomy
-
Pataraia E, Billingsley-Marshall RL, Castillo EM, et al. Organization of receptive language-specific cortex before and after left temporal lobectomy. Neurology 2005; 64:481-487. Interhemispheric functional reorganization of language-specific areas may occur in patients undergoing left anterior temporal lobectomy even when the resection does not directly impinge upon Wernicke's area.

-
Pataraia E, Simos PG, Castillo EM, et al. Reorganization of language-specific cortex in patients with lesions or mesial temporal epilepsy. Neurology 2004; 63:1825-1832. This study demonstrates atypical language lateralization in mesial TLE and intrahemispheric language shifts in lesional TLE.
